New Patients

Welcome to the Department of Rehabilitation and Sports Medicine at UConn Health. We appreciate the opportunity to treat you. Our mission is to provide compassionate, high quality care in an efficient manner.

Our clinical staff consists of physical, occupational, and speech therapists. We treat a wide variety of patients and diagnosis, from sports injuries to complex medical conditions. We have specialty programs in aquatic and hand therapy.

As part of the team at the John Dempsey Hospital, our staff works side-by-side with the physicians to not only care for patients, but to also improve patient care through research initiatives.

Scheduling

We understand that your time is valuable. Your evaluation and treatment appointments are times reserved especially for you and are part of a full schedule for our therapists. If you know that you will be late by more than 10 minutes, we request the courtesy of a call to our office, as we may need to reschedule your appointment. We also request that you provide at least 24 hours notice for any cancellations.

Please complete all paperwork prior to your appointment. This will allow us to utilize your treatment time efficiently.

Insurance Information

  • Please notify us immediately if there is a change in your insurance.
  • In addition to your physician’s referral for therapy, please provide any other pertinent information you may have from your physician (reports, office notes, etc.).
  • We participate in a wide range of insurance carriers and have staff dedicated to obtaining your insurance authorization. Almost all insurance companies require patient co-pays for rehabilitation services.
  • We make every effort to assist you in determining your coverage, however, it is ultimately your responsibility to be familiar with your policy’s benefits.
  • Assistive communication aids are available to those with limited English proficiency and interpreter services for persons with impaired hearing.

Items to Bring with You to Your First Appointment

  • Referral for therapy.
  • Picture ID.
  • Insurance cards.
  • Completed information from Welcome Packet.
  • Any records, reports, etc. from the referring provider.
  • Appropriate clothing to allow the involved area to be viewed by therapist (i.e., tank top for shoulder, shorts for hip/knee, etc.).
  • Braces, splints, or other pertinent assistive devices.

Just a Few Other Things

Please keep communication open with your therapist about your treatment, progress towards goals, insurance authorization, and how you should or need to schedule your appointments based upon your diagnosis and other commitments.

Our goal is to exceed your expectations for your rehabilitation experience. You may be sent a survey after you have been discharged from our services. Please do your best to return the competed survey, as we greatly value your feedback.
